The Loch Lomond Group has revealed that back in June 2016 60,000 litres of Scotch blended whisky with an estimated value of £1 million leaked from a storage vat at the groups Glen Catrine bonded warehouse storage facility and bottling plant near Mauchline in East Ayrshire.

I wonder if it was human error or equipment failure.

Yes, I heard about this accident. That could have filled a lot of Glencairn glasses to say the least. Actually, according to my calculations that would be 2,028,841.4 glasses with an ounce each. To be more specific, every person in Glasgow and Edinburgh (parents could drink their offspring's drams of course) could have a dram and that would only account for about half of the total ounce of whisky glasses available.

That is a lot of whisky to go down the drain before being noticed.
